Transition Navigator Trial (TNT)
Summary
Transition to adult care is a challenging and complex time for adolescents and young adults with chronic health conditions and their families. Many struggle to adapt to adult health care settings and report difficulties accessing needed services to manage their health conditions into adulthood. Limited studies to date evaluate the effectiveness of interventions to improve transition care across multiple health care settings. We propose to evaluate the impact of a patient navigator service to coordinate transition to adult care in Alberta. The patient navigator is a trained social worker who will support the patient for up to 2 years after they transfer to adult care by preparing them for transfer, addressing barriers to health care and coaching self-management skills.
Eligibility
Eligible ages: 16 to 21
Inclusion criteria:
1 - individuals aged 16-21 with chronic health conditions
2 - expecting to transfer to specialty adult services within 12 months
3 - planning to remain in Alberta through the following 24 months
Exclusion criteria:
1 - already enrolled in another study with a patient navigator service
2 - transfer to adult care will not happen within the period of the trial
3 - participant, parent or guardian cannot consent in English
4 - participant does not live in Alberta
