PAUSE-Patient Sick Day Event Needs Assessment Survey
Summary
The purpose of this survey is to better understand patients experience with sick days, sick day management, and how they use the internet and other electronic health tools to manage their illness.
Patients will be asked questions about what they do with medications during sick days (e.g. when feeling unwell, beyond how they feel on a normal day), where they seek care and experiences with managing your chronic disease(s).
The responses will help us develop an e-Health tool aimed at improving how people with chronic conditions manage their health and medications during sick day events.
Eligibility
Eligible gender: Male, Female, Transgender, Other
Eligible ages: 18 to 110
Inclusion criteria:
1. Adults over the age of 18
2. Diagnosed with type 2 diabetes, chronic kidney disease, hypertension, and/or cardiovascular disease
Exclusion criteria:
1. People under the age of 18
2. Diagnosed wit type 1 Diabetes
3. No diagnosis of chronic disease.
