Calgary Stroke Program Register
Summary
The purpose of the Calgary Stroke Program Clinic Register is to determine the risk factors responsible for recurrent stroke and your health utilization over the next 10 years. We will obtain information about recurrent events by tracking the use of health care services (including hospitalizations, emergency room visits, home care service use and admission to care facilities). Patient chart will be reviewed for risk factors and test results. This information will help us determine which vascular risk factors are most responsible for recurrent stroke events. To obtain this information, we will use your personal health care number (PHN). Your clinical data will kept securely for approximately 10 years.
